AKC PupPals Child Request Form
Thank you for your interest in the AKC PupPals Program! This program is designed to provide a note and photo of a canine friend to children in need of uplifting. If you know of a child that would enjoy a PupPal, please include their information below. Please note: Out of respect for a child's privacy, we ask that only parents/legal guardians input the information below. If you are a teacher, family member or friend of a child in need of uplifting, please send this link to the child's parent/legal guardian.

About the Child
Please provide anything we might need to know so that the PupPal can mention it in the letter (i.e. what grade the child is in, their favorite color, etc.). Please note that sharing the reason the child needs a PupPal (i.e. illness or difficult time) is completely optional and not required.
